Please submit an authorization form signed by the consumer, demonstrating your authority to act on behalf of the consumer by replying to the automated email sent within 7 days. Please ensure that the form you submit does not include the consumer's social security number, driver's license number, or any other sensitive information. Maverik reserves the right to deny requests by an agent if evidence of authorization is not submitted.
To complete your request, we require validation of your email address as well as verification of your identity and your relationship to the minor on whose behalf you are submitting the form
You must confirm by replying to the verification email within 7 days of receiving it. If we do not receive a reply containing the necessary documentation and answers to security questions within 7 days, your request will expire and you'll need to resubmit the form. If you don't see the confirmation email in your inbox, please check your Spam or Junk folder before resubmitting.
